Rename a place after something happens there and still find its old name
After the flood the settlers call the camp by a new name, but old notes, maps and elders still use the old one. Here is how to keep every name a place ever had, say which one is right for a given year, and avoid mixing up two places that end up sharing a name.
Why this works
A name is a label, not the thing. Keeping one fixed identity and a list of names with the year each began lets every old map and old tale still point to the right place, while the name in use today wins when two places share a word.
When not to use it
Skip it in a short game where nothing is renamed, or where a rename is just a change of the label you show. Then a single name per place is clearer; use this only when old names are still spoken or written somewhere in the story.
