Hand over a companion's items when they leave or die
A companion walks off in anger or falls in battle, and with them goes the key you needed, or the sword you only lent them. Here is the rule that decides who gets what back, so no quest is stranded and nothing vanishes with the person.
Why this works
Things belong to someone, and a departure should respect that. Lent items go home, items the story needs always return to the player so no chapter hangs on a person who can walk off, and only what is truly theirs stays with them.
When not to use it
Skip it when losing the items is the drama, such as a betrayal where the thief keeps the prize, or when the story never depends on what a companion carries. Then a plain rule that nothing moves is enough.
