Nudge loot toward what a starving or thirsty player actually needs
A player dying of thirst who opens three chests full of bandages feels the game is unfair, but a chest that always has exactly the needed item feels fake. The nudge has to be strong enough to save a run and weak enough that nobody notices it.
Why this works
Bad luck feels unfair when it hits someone already in trouble, while help that is obvious feels fake. A push that grows slowly at first and strongly only in real need rescues runs without anyone seeing the hand behind it.
When not to use it
Skip it in a game about scarcity, where being short of food or water is the challenge itself, or in a game where players compare loot. Offer more ways to craft the missing thing instead of bending the dice.
