Keep what the character really saw apart from what they imagined
If a scare shows a ghost and the story then records that a ghost exists, the mystery falls apart. Here is how to keep a ledger that remembers where each clue came from, so the notebook and the final accusation only use what is real.
Why this works
A player can only reason about what the story lets them trust. Keeping every sighting together with whether it was real means the journal can show doubt honestly, and a late reveal that it was all imagined still has something to show for it.
When not to use it
Wrong when the character themselves cannot tell real from imagined, so the game should not tell the player either. Then show every clue the same way, and let the story decide later which ones were true.
