I renamed the sword and the AI changed the swordfish and the sword of dawn too
A simple replace-all is a blunt tool: it finds your word inside longer ones and in other capitals. Your AI can first list every place the name appears, sort them into sure ones and ones to look at, and warn if the new name is already taken.
Why this works
Whether a match is the same thing depends on what surrounds it. Looking at the characters before and after a match separates the item itself from a longer name that happens to contain it, and a plan before the change makes the risk visible.
When not to use it
In prose, a word may be meant as an ordinary word, so a plain list of places is not enough. For story text read each line before changing it.

Say this to your AI
Use the Spielerando entry "I renamed the sword and the AI changed the swordfish and the sword of dawn too" (read_entry rpg-rename-item-everywhere-safely) in my game. Build it step by step, and show me how you checked that it works.Or skip this and connect the MCP: your AI then finds all of this by itself.Connect the MCP
